On calling up he was 5 ft 1½ in tall, chest 35½ in fully expanded with 3½ in expansion, medium complexion, brown eyes and black hair. Distinctive marks: large scars on breast and mole on left side; noted as underweight.

Service file is explicitly marked 'DESERTER' / 'Illegally absent.' Pay certificate records S.O.S. as deserter and nullified assigned pay from 23 October 1918. Proceedings state illegally absent since 23 October 1918 and struck off 13 November 1918; confirmation dated 2 December 1918. No Good Conduct badges, medals or decorations were recorded in the proceedings. Page 17 is an unrelated parchment certificate for Arthur Herbert Lindsay Richardson (Strathcona's Horse, South African War) and must not be attributed to Laviolette. A punishment card also records a pre-enlistment Military Service Act enforcement episode: absent from 10 November 1917 to 7 September 1918 (311 days), with 28 days detention and forfeiture of 311 days pay (D.O. 262, Ottawa, 12 September 1918). Because this predates his formal enlistment on 17 September 1918, it is preserved as research context rather than as military-service chronology. A lower entry dated 27 September 1918 names Laviolette, but its full administrative meaning is not sufficiently legible to expand.
